Hi,
I don't have any tatoos myself, but I have Mitral Valve Prolapse, and I have to sign a waiver even when I go to the dentist. I think the waiver is a reasonable request, but the doctor's note??? I think that's a bit too much. I agree a lot of doctors wouldn't write it... at least I don't think any of mine would... I would think that a waiver is enough... Maybe you should try to find another artist.
