I've done in manually once when I couldn't find the serter. It wasn't painful, just a bit of a pin prick, and there was, thankfully, little bleeding. However, the sensation of doing it was awkward. The long, heavier gauge needle along with the 45 degree insertion angle made the process interesting. There was a lot more resistance than I was expecting and a good bit of pressure was required.

Overall I'd do it again if needed but I'd rather use the serter.
