I have never had a real milkshake with high sugar ice cream so one made with Breyer's low carb ice cream tastes great to me. One-half cup of Carb Smart has 10 carbs. I use one cup and some whole milk. I let the ice cream get a little soft so I can just stir it all together with a spoon. I make sure the milk is very cold. The whole thing has about 25 carbs. I have read that whole milk is less likely than skim milk to make you spike. It is not a big shake but it satisfies my taste for one. In the summer time i use Breyer's vanilla and mix in soft fresh peaches or presweetned berries, in which case I don't use any milk. The juice in the fruit thins the mixture enough that I can stir without adding milk.

Different strokes for different folks. LOL!
