2. At what age were you diagnosed? 12
3. What type of treatment are you using? Insulin - Novolog and Lantus
4. Do you control it yourself (testing, insulin)? If not, who does? Yes
5. How many times a day do you test yourself? 4-7
6. Does it affect your confidence level at all? No
7. Are you more careful with your diabetes when you are out with your peers? No
8. How many times a week do you exercise? I'm not right now, hopefully will start back up in the next 2 weeks.
9. How many times a week does your blood glucose go low? every night, but I am in the middle of insulin therapy because I am getting on the pump in a month.
10. How many times a week does it go high? maybe 3 to 4 times
