That seemed strange to me also. When I finally come around and decide to do something about my D, he doesn't want to give me the things I need to do it with. Also, it's just a piece of paper he has to sign. It's not going to cost him anything, and if it makes the patient happy, then sign it. What harm would there be in doing that?

Well, I haven't been to his office since this has happened. This was all done by phone, and with his office people, not him.(well, except for one e-mail to him.)
