kidney function not 100%,help me!

are there any diabetics here that have been told their kidney function is not 100%? My last blood test results showed a high creatinine level,it has been the same for 2 years. Does anyone know about this and what have you done about it? Are you seeing a kidney specialist? or your regular doc? what can you do about your kidneys to help them to function better? any tips? I'm getting freaked out about it and I need some help here. It would be deeply appreciated. thanks in advance.
