If you do get permission, you might want to modify the questionaire to be useable. It's hard to circle on the internet.

As a side note I'd sit out, as without a hint or background into the potential quality of the product/service, (or the intentions of the company/people involved) I have no way to judge whether the information I provide would be used for good or evil.
