I recommend frequent and vigorous exercise. Variety is good but if less variety still works for you then fine. You want both, strength training and cardio. Hit the biggest muscle groups for greatest effect but be sure to work them all from time to time. If you're not sweating, you ain't doing it right. Be patient, don't expect quick results. Find a routine(s) that you can sustain long term.

Mostly I just skip bread. Sometimes I use low carb wraps for a substitute. I agree that sourdough or rye/pump are better than most. Making your own bread is another option, go for grainy peasant type recipes. The whole grains, or less pulverized grain flours will have lower glycemic indexes which is what you want. Ultimately testing BG is the only way to know what works best for you.
