However, especially if you are eating less than you used to, you should be checking your blood more.
The Lantus might still be perfect, or it might be way off ... there's no way for you to know without checking your fasting/pre-meal levels.
If you are on the "right" amount of Lantus, your pre-meal levels should be in the 4.6-6.0 mmol (82.8 - 108 mg) range ... if they are running higher than that, you may need to increase the lantus; conversely, if they are running lower, you should decrease it.
You are correct in stating that the amount of rapid acting insulin should be calculated on the overal number of carbs you eat, but fat and protein count too ... they take longer to hit.
if you eat alot of fat in a meal, you may need to take one small does of insulin 10 minutes before the meal, and another correction an hour later!
Again, the exact numbers need to be calculated by checking your blood 1 hour, 2 hours and/or 3 hours after eating.
Only you can know exactly how much insulin you need ... the doctors are there to help you, not dictate to you ... but you definitely need to test regularly.
