"...As far as the Joslin consult, I spoke to several of my Diabetic Consultants here and they concur that if your priority is to go to Joslin no matter what, you should change your primary care to a non HVMA provider. (We have many wonderful endocrinologists some of whom came to us from Joslin) I'd be happy to refer you within HVMA if you'd like.

My PCP does not support me going to the best care

If I had cancer, I would not expect them to treat me as they are not specialist. They are not diabetic specialists so I would not expect them to give me the ultimate care either.
