jesus | Probably the best way to do it is to camp out in the reception area of a diabetes clinic and ask patients to fill out the form. Or maybe you could get a CDE and/or endocrinologist to work with you on it - the questionaire is short enough.

Pat | that is a great point. the conclussions of the study might head in that direction. We'll see. I will be glad to share my results with this forum since it has been so helpful.
