IDF guideline for the management of postmeal glucose

I found this an interesting read (apologies if this is a re-post)...
Quote:
Until recently, the predominant focus of therapy has been on lowering HbA1c levels, with a strong emphasis on fasting plasma glucose. Although control of fasting hyperglycaemia is necessary, it is usually insufficient to obtain optimal glycaemic control. A growing body of evidence suggests that reducing postmeal plasma glucose excursions is as important, or perhaps more important for achieving HbA1c goals.

Well I pretty much hit the less than 140 2 hours after meals.
I'm a "dawner" so the fasting number doesn't work got me.

Thanks for posting this, Frank! Amazing information, here, and very encouraging, since I usually hit the under 140 PP mark. That makes me feel I'm really doing what I need to be doing.

That tangle all around my.... er .... knees

The hours I'm hooked up? All twenty four
And that's it for now until evermore
But I disconnect for up to an hour
For wonderful fun (and sometimes a shower)

And when I 'suspend' it, it plays Barry White
And my wife knows she's in for one heck of a night
But only an hour of that night is with me
As an hour is all I'm allowed now, you see...

This was a good summary of what many of us have been moving toward...I first heard the "140" theory from my endo about 4 years ago.

I am happy they cautioned about over-bolusing at one hour for a higher number...those on insulin as the insulin is designed to work 3-4 hours.

They did bring the Amlyn (Simlyn) meds into the study...but I'm still not convinced that the percentage improvement is worth the trouble. But, that's not the purpose of the study. For some, delaying the carbs against insulin action is a caution in itself and should be discussed with doctors.

I also agree with the summary that post-meal numbers don't necessarily show up in a low A1C (just as wide swing in numbers don't, too)...I think for improved health...these post-meal numbers are a great tool.

I most appreciated the subject of the GI "load" in the study.
