Question If you had diet-controlled GD but started insulin...

...what factors did your doctor/peri use to determine the switch? Was it something along the line of "oh, five really high elevated numbers, time to switch?" Did they give you some time to change things up and work it out first? I know some physicians are more aggressive about gestational treatment than others, but I'd just like to have some kind of general idea.

I've only had a couple slightly elevated numbers this week (two very much explainable by outside factors, one not so much), so I'm not too worried about this now. But it got me thinking and wondering about it.
Diagnosed with borderline gestational diabetes (A1) at 25 weeks. Due May 6th. Gave birth to son, Oliver Charles, May 4th. Struggling with higher postpartum numbers.

If they are fasting, mine would only tolerate a few. Post prandial, they didn't have to be very high, just high a few times. There was no hard, fast number of highs that was acceptable. I imagine it depends on how high your normal values are in relation to the spikes. But they do have a very low tolerance for higher fasting levels.

I went on insulin with #2 when my breakfast PP were 130 to 140 at 1 hr a few times; my fastings were fine at that point. With number #1, I got put on glyburide after a few 2 hrs at 140-150; definitely not 5, maybe 3. But there was no rule; they looked at the whole picture. One or two spikes a week shouldn't sink you if the other numbers are great.

My doc and I came to the mutual decision of giving me Glyburide because despite my best efforts diet was not helping me control my fasting glucose. I'd be fine all day and then wake up with glucose of around 105. They want it at 95 or below, but things I'm seeing today make me think 90 or below would be ideal.

With me, a spike generally makes me take a closer look at what I ate. I use the insulin as an aid not a replacement for diet and exercise. I have had two spikes since starting the glyburide a couple of days ago and every time I take the 2.5mg of glyburide my glucose plunges and I get ill so I don't think upping my dosage would be the best option. I'm just learning balance.

    Luck to you.
