I currently take my Lantus daily at 9am but am considering swapping to evening - can somebody tell me how I go about this to enable it to be a smooth process?

You could just cut your morning dose 50%, and add an equivalent dose in the evening, making sure that you test and correct for an overage. Then, after a few days, cut the other 50% over.
If you were going to do this, I'd suggest you pause a while at the morning and evening dose stage, to see if you get any benefit from the split dose. Many people find that their Lantus doesn't really give 24 hour coverage and do better on split doses.
If this were the case with you, you'd see an improvement in youir BG for the period of, say, 7->9 am. when you are on split doses.

The easiest way to switch times is to split your current dose by 50%. Then at your new evening dose, give 50% less the first time. Now, keep in mind you are going to run high during this so you will need to correct every so often with your fast acting insulin. Then the following day go to the full 100% dose and continue life as normal. It should go like this...

9 am - Give 50% of Lantus dose
Lunch meal - Test and give correction factor as needed
Dinner Meal - Test and give correction factor as needed
9 pm - Give correction dose as needed and give 50% of what you gave in the morning
9 am (following day) - Test and give correction factor as needed
Lunch Meal - Test and give correction factor as needed
Dinner Meal - Test and give correction factor as needed
9 pm - Give correction dose as needed and continue on like normal

That would mean at 9pm she would give 25% of her original morning dose according to those instructions Kev. I think you mean the same dose as her morning dose, per that day(or 50% of her original morning dose at that time). <I am such a nitpicker>

Your endo will probably tell you the exact same thing, but you should ask a doctor about this. They deal with greater populations of lantus users. Granted you ultimatly make the decision how.

I seem to be suffering from DP which is why I am considering taking my Lantus at night instead. For instance I went to bed with my bs at 7.7mmol/L, tested at 2:45am & I was at 8.2 mmol/L & then at 7:45am I was at 13.8mmol/L!!!! Or may be the Lantus is wearing out? Still, either way I think trying the dose at night first is worth a shot.

My consultant did question why I take it in the morning & I told him, calmly, because I was told to!

So, basically to clarify I should:

9 am - Give 50% of Lantus dose
Lunch meal - Test and give correction factor as needed
Dinner Meal - Test and give correction factor as needed
9 pm - Give correction dose as needed and give 50% of what you gave in the morning

9 am (following day) - Test and give correction factor as needed
Lunch Meal - Test and give correction factor as needed
Dinner Meal - Test and give correction factor as needed
9 pm - Give correction dose as needed and continue on like normal i.e take 100% Lantus?

I have a GP appt Monday week so I can run it past him before I make any changes.

I would change the quote to read:
"and give the same dose as in the morning (50% of the original)". This may or may not be what you meant. The idea is to be taking the same total Lantus dose in that day. And, yes, the next evening, the full Lantus dose.

Still, either way I think trying the dose at night first is worth a shot.Georgia, I think it's very possible that the Lantus is running out and it might be worth your while to try what seacomp suggested: keeping a split dose to see if you get good levels that way. If you don't and go straight into the one evening dose and you find your bg starts to rise 3pm-9pm, it probably means Lantus isn't lasting 24 hours for you and it would be in your best interests to try splitting doses. It could be a combo of both, too: a bit of DP and Lantus running out.

You have received some very good advice on how to switch your Lantus from morning to evening.

I have been on Lantus for several years. I am a type 1. I like the Lantus. I take mine at 9:00 p.m. and only once a day. Lantus seems to last at least 24 hours for me.
