Work has been busy and Thursday I stayed late. We had ordered some pizza, from a pizza place across the street. This is New York City pizza, not wimpy little Dominos or other national fast food chain pizza.
I looked up nutrition info for pizza on Calorie King. For generic cheese pizza, original crust, they list a slice as weighing 3.6oz. What self respecting pizza weighs 3.6 oz per slice?! I weighed my 2 slices - 15 oz! So I used the numbers for 4 slices of what Calorie King had listed, which came out to:
carbs: 134
fat: 39
protein: 49
Let's not even think about the calories right now...!
I had never used the TAG system for calculating a bolus but I did a quick search to see what people were doing and followed an example I used. I don't remember the numbers now, but I ended up entering 174g as my carbs. I did a 50/50 bolus with the square portion spread out over 5 hours.
I ate the pizza at 7:40pm while still at work. My highest reading over the next 5+ hours was 98, lowest was 82. I woke up around 5am this morning and was 91. I was only 78 when I woke up for good at 8:30.
I can't believe I actually did real NYC pizza! My first time eating it on the pump.

Great job, Liz! Such a good feeling when we finally get that extended bolus thing right. I'm still learning, but I'm getting better.

Yes, and then about 15 minutes after the pizza I had a Cocoa Via chocolate crisp snack bar, an additional 15g of carb. Total count for dinner was 189g, definitely way more than usual! I have gone over 100 a few times when eating pasta.
Eating pizza is not something I plan to do often, at least not real pizza. Those 2 slices were probably around 1,000 calories!! I can eat a LOT of more nutritious food for those calories. But, it is nice to know that I can do it if I want/need to.

Anyhoo, congrats! I think all ya'll that do the boluses for certain foods and such must be math wizards. I don't know if I could do it!!

aahh you had to throw that line in about real NYC pizza huh?? that has got to me one of the things I miss most about living in NY, well that and real chinese food, and people that know how to drive :) ......

From what I looked up online, I calculated the carbs to be 134g. The extra 40g were added on to account for the fat & protein. The 5 hour extended bolus was also done to deal with the fat & protein. These two slices weighed nearly an entire pound!!
My insulin:carb ratio for dinner was 1:28.

Yea, I thought you might say something about there (although it may seem out of wack for you, it could be perfect for you). I eat 2 slices at about 75-77grams, but my insulin:carb is 1:11 and I do pizza at 50/50 combo for 3:30 hours.
If the formula you have now is working well in the long term--there is no sense in changing it. I would check 15, 30, 60, 90, 120 minutes (the early times are good to check and see if you need more/less than 50% up front) after eating/bolus and actually see how your combo bolus is working for you. It's pretty cool when you can see the protien kick in an hour or more after and the combo bolus starts working on the rest of the food. The more you test and play, the more you learn about how your body reacts to specific types of foods: fatty, protien rich, sugary sauces, etc.
It's a fun game sometimes.

Okay, so let me ask... Since I'm not on a pump yet, but am on MDI, how could I split up my shots so that I can have pizza when my family decides that's what's for dinner?
I'm not a big pizza lover, but occasionally I do get a craving for a slice. :)
Oh, I'm 1:5 on Humalog.

You could try something simple at first. For example, take your normal bolus amount but split it into 2 doses. Take half of your insulin with dinner and the other half 2 hours later.
I bolus for carbs and protein (equivalent carbs) when I do pizza, YMMV.

Wow, congrats dude. My grandmother is diabetic, and she is aching for "real" food. you think its safe if I let her have a slice or two should she want one? She is under medication and some supplements not taking the injectibles. I'm sorry I really don't know the terms you use, i forgot to mention that I am not diabetic. I join forums to help my grandma. And mostly she yells when we serve her food because she says we don't feed her enough (i think what she says is we don't give her what she wants).

Oh, I'm 1:5 on Humalog.
I'm on MDI and do a split dose for pizza. I do 60% before eating and 40% plus a correction for whatever my blood sugar is an hour and a half or two hours later, and it works really well. If I eat pizza for dinner I'm fine at bedimte but then I do shoot really high overnight, but for meals other than dinner the split dose works well.

do you just add the protein grams like they're carbs, and then bolus for the total?

I think I counted 60% of the protein as carbs, and a percentage of the fat as well. I then added that to the actual carbs and came up with the total number I was going to bolus for.
I keep meaning to try Chinese food again one day and see if I can figure that out.

I'm on MDI and last night i had some friends over and we ordered a couple pizzas. I thought, hey, might as well try it out (first time since diagnosis). I had read on these forums the different combo boluses that people use to cover pizza but I thought I would see what would happen if I did one regular shot of NovoLog. At dinner I do 1u per 30g carbs and i counted each slice as 30g. I had 4 slices so I took 4 units. My bg was 74 before eating (4:30 pm) and 117 two hours later. I then tested every hour or so afterwards and my highest was 97. I went to bed at 1am with a bg of 88 and had my regular snack - 8 carbs and some protein and woke up at 9:30 a little low, at 67. I was surprised that I didn't spike at any point. I'm now thinking maybe I went high in the middle of the night but it sure would have had to take awhile, about 8 1/2 hours..

You seem to be in your honeymoon phase, so your pancreas is still there to help you out a bit.

i don't think that's the case. my insuling requirements are 27 units of lantus each day and for meals, between 1 unit of novolog per 15g carbs and 1 unit per 30g carbs depending on the time of day. if i neglect to take enough insulin i will go high. i have talked to my endo and DE and neither think i'm in a honeymoon phase. i think i just got lucky with the pizza. :)

Was it pizza from a real pizzeria, or from a fast food chain like Dominos? I fond fast food pizza is easier to do. Mostly, though, I stick to making my own on tortillas or pita or flatbread or english muffins.

it was more fast food style, from Prime Time Pizza. I guess that will be my primary source of pizza from now on.

I work for Pizza hut, and therefore eat pizza more often than I should. I usually find that taking the humolog immediately before the pizza works fine, and I would expect to be back down to normal levels within 2 hours. Seems simple, but the fact that you can take a bolus to cover the pizza does not make it good for you , ie you will put on wieght the same way as non diabetics do!

How Would That Be!!!! I wish I had a ratio like that! I do 1:10 and I read another person here that is 1:5. If I had a 1:30 ratio I would spend half as much money on e insulin....anyway. I too had pizza last night, first time with the pump. BG was ok but could be better, but not to bad for the first night on the pump ;-P

i'm experimenting with bolusing for high fat meals as i noticed i go up to 12 hours later for chocolate and one time an indian restaurant meal.
is 50/50 a good place to start?

I don't do bagels too often anymore. Last bagel I had, I weighed it and figured it was 85g of carbs. I had it anyway with cream cheese & lox but have stayed away since. Not even so much because of the carbs but because of the calories.
Frozen/packaged bagels just aren't the same.

The 134 carbs is enough to say that this will keep you high for a long while and this is whare i get to do "Bridge Bolusing"
