I'm looking for comparitive data for Apidra and Humalog. Can anyone point me to data that compares one against the other in terms of speed of action? A comparitive graph showing impact over time would be really cool...anyway - any data from a reputable source would be appreciated...

OK - from what I can find on the Lilly website (Humalog) and the Aventis website (Apidra) it appears that Humalog reaches it's peak effectiveness at 1 hr 45 min, and is still at 80% of it's max at 2 hrs. Apidra reaches it's peak at 1 hr. and has dropped to 50% effectiveness at 2 hrs. Both websites had "Time/Activity Profiles" for their products.
The reason I'm questioning this is that I can't get rid of my post prandial spikes (I'm pumping Apidra and shooting Symlin) and I'm wondering if Humalog, with a longer peak, might help...
Anyone have any thoughts or experience in this regard?

Yes, the longer action profile would help you, or if you are pumping insulin, you'll need to tell your spread out your bolus over a longer period, say a couple hours, rather than taking your entire bolus over a couple minutes.
You could get equal results out of both insulins so long as you figure out the correct dosing schedule and amount.
My idea would be to stick with something that I trust, and then work with it until I get it to work for me. But we're all guinea pigs, so change if you feel like it, but don't change your ideas too quickly before you make a discovery that works for you (sort of like how insulin would have been discovered much earlier if nobody had given up!).

Ok, so when I first went on Apidra I noticed my post prandials were SO MUCH better than when I was on Humalog. Then the last 3 months I started having issues after some meals. I wore the CGMS for a week and we found that if I eat a meal with more the 45 grams of carbs, and bolus all my insulin upfront (which I always did) I spike 2 hours later. So, now when I eat 46 grams or more in a meal I do a dual wave bolus over the 2 hour period and that did the trick! I like that Apidra is absorped faster b/c it allows me to dose it after I eat (which is great if you end up eating less than you thought) and don't have to worry about spiking, as long as I'm watching how many carbs I eat. It's supposed to be more physiologic b/c it's in and out so quickly, but it's still not perfect. I don't know if that even helps.....

I use Humalog with really no complaints. But I have looked at possibly changing to apidra or novalog.
Really the only reason would be because there are times where I seem to spike during the 3rd day wearing my set. I attribute this to degraded humalog (realizing many issues could be causing this).
I noticed that the apidra-makers state that apidra should be used for 48 hours or less. I didn't see a recommendation for humalog (yes-I should already know this) or novalog with respect to maximum time in a pump. Any help would be appreciated.

Yeee Hawww! I tried an extended bolus with a 70/30 split over 2 hours and I think I nailed it!
THANKS A MILLION for the advice. I've been working on this for a long time!

Has anyone looked at or overlooked the information provided in the "Pumping Insulin" Book 4th Edition?
Does anyone have any graphs on Symilin?
