Dude, what do you eat? Seriously, I didn't know that cholesterol numbers that low were humanly possible. I don't have high cholesterol -- my numbers are well within ADA targets -- but wow. The only thing there that you might want to work on is bringing your HDL ("good" cholesterol) up above 40. Chow down on nuts, avocados, and throw a dash of olive oil in your food. If you smoke, try to quit. Exercise helps raise HDL numbers, too. Otherwise, wow (once again).
As for the glucose numbers, all I can tell you is what happened with me. When I presented, my glucose level was over 1,000 mg/dl and my A1c was 13.4. I started using two kinds of insulin right away (Lantus and Humalog), and gradually, over the course of several weeks, stopped taking insulin altogether -- for now. But by the time I stopped taking insulin, my glucose readings were in a pretty good range most (but not all) of the time. In addition to insulin, I started out on glyburide (which squeezes more insulin from your pancreas); after several months, I stopped taking glyburide and switched to Actos (which increases your body's sensivity to insulin). And that's where I am today, almost one year after diagnosis.
I don't know what other tests you might want or need, but you probably do need to talk to a nutritionist or Certified Diabetes Educator about how to structure a meal plan. You should probably also tell your doctor what your glucose levels are. It takes a while for them to come down -- at least that was true for me -- but after three weeks, readings of 250-400 ought to be extremely rare, not the norm. It just means that your insulin needs adjusting, at least until the metformin kicks in.
Good luck, Troutman.

One of the symptons of high blood sugar is fatigue. I probably had diabetes for well over two years just thinking I was getting old, cranky and started having problems with my teeth.
Once you get your BS down to around 100 you should be o.k.

While my experience will certainly be different from yours as a T1, I do find myself irritable, tired and irritable when my blood sugars run high. If you continue to run high with the changes to your medication, you may want to consider sending your doctor a fax of your blood sugar results (and diet) every few days so that treatment decisions can be made more frequently. You will have more energy once your blood sugar returns to the normal range and you will look back at this period in awe of how resilient you were.
