View Full Version : Start Pumping Tomorrow - Quick question

Well my appointment to start insulin is tomorrow. I forgot to ask my CDE how to prepare in terms of meds. Currently on Lantus (evening) and humalog. Didn't think to ask if I should take my normal dose the night before or skip it, since I will be connecting to the pump around 7:30-8am.

Right now I figured I would skip it (usually take it around midnight or so) and just do a correction before bed if necessary and in the morning when I wake up.

Just wanted your opinion on this. My CDE is nice, but hasn't been really up on telling me what all I need to bring to my appointments. I actually had to ask for a script for insulin. They are switching me to Novolog, saying that humalog tends to stick to the tubing. But then she didnt seem like she was going to give me a script.

Well I'm no expert, but what I would do... and what I did....

Take my "usual" Lantus dose and just note when you took it. During your start up appointment, tell them " I took my usual Lantus dose of 'x' units at 'y' time. ".

I had a pretty flat consistent Lantus action over 24 hours and so they had me hook up to my pump, during my appt, but turn basals down to zero until the end of the 24 hour period, then set them according to our agreed rates.

I then checked my BS every hour for like 2 days (my own obsessive compulsion). I was fine. Your mileage may vary!

Personally, I would skip it and take correction boluses.

If I remember right, they had me take 1/2 of my Lantus at bedtime instead of the whole dose.

I think MM advises to take it because when you program a basal for the first time, it won't start until midnight.

I was on Humulin N at the time. When I went on was in the afternoon. I/we just reduced the basal rate down and the next morning just went back to the normal basal rate.

ah, that's not really true, lili. when you program your pump, the basal starts as soon as you confirm the basal settings.
the first day is no different than any other day as far as when the basal begins. it begins IMMEDIATELY

I think you just misinterpruted it. It means the started time for when you start to program it. Like, the first "block" of time that you program will be 12-4am, or whenever it is. You can't started your profile at 4pm or whatnot.

Mine had me take the usual dose, but started the pump with a temporary basal that switched over to the regular basal at 9:00 PM. I think they can figure out what to do with whatever you walk in with. I did tend to run VERY LOW that first afternoon so not taking it sounds like the better bet to me. Good luck! Expect a long learning curve with this. But it's all worth the effort. Even in the short time I've been doing this I wouldn't want to trade back to the 5 or so shots a day I was doing, or the larger corrections after larger highs, or the inability to do tiny corrections, ... .

When I started I was told to take half my lantus dose the night before then run my normal pump basal. I was 40s the entire first day. I recommend either skipping lantus and correcting as needed, starting pump when you normally take lantus, or taking normal lantus dose and using temp rate to get you through till it wears off.

well so far so good. I love not taking shots.

I didn't take my lantus, which apparently was correct.

I took two shots of Lantus and took my evening shot and then my morning shot the day of my training. My training was in the evening and my CDE set a temp basal for I don't remember how many hours. She told me not to worry if I woke up high the next morning and to just correct with the pump.
