View Full Version : Novolog :(
Recently diagnosed. I currently take 13u Lantus and was just started taking Novolog on Monday. I take 1u per 15 carbs prior to eating. The problem I am having is my BS level afterwards. Since I just started it, I check one hour after injection, then two hours, then four hours. The first two hours are good, below 110 usually, but at four hours it's close to 200. Has anyone had this happen before? I just opened a brand new bottle of Lantus. It's as if the Lantus isn't even doing anything now.

I'll bet you need more Lantus. It sure sounds like your basal rates are off. Keep good records and discuss with your endo asap.

I agree with xmenace.

That's what I was thinking, I've been increasing it (prior to Monday) for the passed few weeks. I didn't want to make a drastic incease and end up messing myself up though.

Most people require about 24 units of basal per day, of course it varies.
What are your fasting (taken right when you get up) BG numbers?
Generatly you start with a low basal dose and slowly increase it until the FBG is at your target.

Between 120 and 150 lately. It's been abnormally high, I'm used to 90, +/- 10. I take my Lantus in the morning, anywhere between 8am and 10am. This morning it was 153. :(

For a lot of people, Lantus does not work for 24 hours even though that's what's claimed. It may be that your Lantus is "wearing off" by morning.
Myself, I split my Lantus and take a shot in the evening and another in the morning. I get better results that way. Many folks in the forum do the same.

Is this with every meal or just your first one?

I understand it doesn't last 24 hours, it only lasts 18 I think. It's just weird now that I've started on the Novolog that the Lantus just doesn't seem to be working like it used to. I never had a problem with high BS after I wake up until the Novolog. I'm deathly scared of taking Lantus at night, for fear of getting too low and not waking up! ;(

It's with just about every meal. After the four hour mark, my sugars are around 200. I'm going to slowly increase my Lantus for the next few days and see how that goes.
If it's with every meal, than I would be inclined to say it's not your bolus insulin, but rather Novolog wears off faster for you than 4 hours. You can determine if your basal insulin is dosed correctly by skipping meals and bolus insulins and monitoring your blood sugar levels every half hour until you can't take it anymore, or until your sugars rise above 140 or fall below 90.
If you determine it's the Novolog wearing off too soon, you can try Humalog or even R insulin...R will kick in slower, but lasts a little longer.

I'll try skipping the carbs and bolus tomorrow, I'll just eat turkey all day ;D, and see what that does. I mean it shouldn't at all rise above 140, correct? The last time I ate tonight was around 6:30 so my BS in the morning shouldn't be high (meaning above my normal morning reading prior to Monday) at all I'm thinking. I'll keep you all posted! I'm hoping to hear from my CDE tomorrow.
Also, happy holidays to everyone and thank you all for your responses! I hope Santa was good to you all!
I dunno, I ate a lot of turkey today and it took me until about an hour ago to get back to normal, LOL.
Good luck, keep us posted.

Just turkey raised your BS? I guess I won't be eating turkey all day tomorrow! Besides, I'll be at work. I should try and do it during the weekend as I'm very active at work. Might be an easy day considering it's the day after Christmas. I do hope everyone wrecks their cars though! I mean that in a safe way.
<<Body shop technician.
J/K about the turkey. For the record, though, Chicken in copious amounts and beef in almost any amount requires me to bolus two hours after eating. Fish, pork and other meats seem to be neutral to me.

"Suck it, Trebek!"
Well it's been almost a month since I've updated you. I think I've got it all figured out! I've increase my Lantus to 18u, still taking it in the morning. I've been doing well since my last post, I think my body just needed to get used to the Novolog maybe. I was having mild headaches and lightheadedness(?) after injection, but it's stopped since. My average BS for the passed 14 days is 100, down from 121 for 30 days, and down from 130 for 60! I hope everyone else is doing well!

What are your 2 and 4 hour post-meal readings like now?

Now I just check after 2 hours, but it's usually lower than 80 because I'm at work. I'm very active at work, so I just eat some Starburst throughout the day! ;D It's not often that I am above 120 though. Tonight I was, I ate Mexican (rice + 2 burritos) and I wasn't sure how much to cover for the rice. I didn't bolus enough though, but 168 isn't bad I don't think.
