View Full Version : appointment with my CDE this morning

I had my regular 3 month appointment with the CDE today. We butted heads a bit as usual over ratios and numbers. I did take her advice and changed my lunch ratio to 1:30. I usually go for a walk after lunch and tend to go low. It's not a basal issue, definitely bolus. So I made that change but wouldn't change my dinner ratio. She was pointing out days in my log book where I've gone low after dinner, but those weren't the most recent entires she was looking at. Lately I've been doing okay at hour 2 post-dinner, then rising before bed. I'm having a tough time figuring it out. I know I need to do some basal testing.

She said that she went to some conference recently and saw the new, smaller Minimed CGMS sensor/transmitter. She said that she immediately thought of me and she was urging me to go on it when it's available. Unless insurance wants to cover it, I won't be able to afford it. Right now money is so tight I couldn't even afford the few hundred $$ it would cost just to upgrade from my 715 to the 722 on the Paradigm Pathway program. And that would just get me the pump, which I wouldn't need without the sensors and transmitter. My pump is less than 2 years old, so I've got awhile before I could even get insurance to cover a new one fully.

My A1c was 6.0, which made me happy since the last 2 were in the mid-6 range. I thought it would be higher, especially since my hemoglobin results have been low and I've read that can cause a false high A1c reading. I asked my CDE about that and she said that my level has been low for awhile - thanks for telling me!! She said it's been in the 11 range and I think 12.3 or 12.5 is normal. She said that I've been slightly anemic since 2005 - that's when my iron level dipped. She put those tests down again on the lab form she gave me today, but I won't have blood drawn for that until before my next appointment in April.

Overall I'm happy. My A1c was better than I expected, my CDE didn't threaten to drop me as a patient if I didn't do everything she said, my blood pressure has remained good after stopping the fosinopril last April and my weight has remained steady (although I'd like to drop about 7-10 pounds still).

I'd love to get my A1c back in the 5 range but then I'd have to listen to a lecture from my CDE. I'd be happy with 5.9.
