I'm still on my Lantus/Apidra regimen. I actually take a lot of Lantus, twice daily(morning and nite or else my blood sugar creeps up) My usual dose is 20 units in the morning (at 8am) and 20 units (at 8pm)
If I don't do any exercise, that keeps my BG flat between meals unless I really pig out at lunch and don't give myself enough Apidra to totally cover it. But I've recently started an excercise programme, and here comes the problem.
If I do my workout in the morning, before or after my Lantus shot, by 16:00-17:00 my BG starts to skyrocket, and I have to take as much as 5-6 units of Apidra to keep it down till my next meal/Lantus shot. It climbes like a rocket and if I don't carry my apidra and a tester, I'll probably end up at 16mmol by 8pm.
So I tried increasing my Lantus by 2 units, and things are a bit better, but now between say 12:00-16:00 I gotta constantly check myself as by BG slowly creeps down. Now comes the other problem. I've tried moving my exercise programme to the evening, 1st morning I woke up at 12mmol, with my evening Lantus shot at 20 units as usual. So I tried increasing it to 22 units last nite, I woke up this morning at 16mmol.
Now why the **** is exercise doing this to my body? (If I stop exercising, BG goes back to normal. And what really puzzels me is the fact that during the exercise, BG behaves normal or creeps down a bit. Why this boomerang spike reaction 6-10 hours after the exercise? Can someone who has experienced the same please give me some advice, as I really don't want to give up my exercise routine, but this doesn't make sense at all to me.

What type of exercise is it? Cardio or Weights?

I have heard someone who had this problem say that they got rid of it by going low carb so that could be worth a try.
I have not had that problem myself, but also don't eat too many carbs. I find it's easier to keep stable.

exercise can speed up insulin absorption.
Do you get very hot when you exercise? Do you stay warm afterwards? Do you sit in a jacuzzi, or a sauna after exercise?
a combination of the above can speed up insulin absorption.
From what you describe it sounds as though your lantus is ending earlier than expected - I'm also wondering whether you're getting a 12h rather than 16-20 hr duration out of your lantus (it does happen in some people)

Another possibility is liver glucose dump in response to the low-ish bg after exercise, which results in high bg some time later. I haven't experienced this myself, but have read of others
who see this effect.

If you think that 20U of lantus is a big dose then i take 44U at night!! So sudgest to increse the dose buy 1U each day and look at your BG's with interest!!.

That is unuseual. Could we say that there is insulin missing?
Meaning when you go to do your workout there is not enough insulin entering your mussels along with glucose?

Can we assume that when you exercise your motabolisum has sped up and you have used up your reserve of insulin?
In short increse the lantus slowly!! :)

What also might be happening is your body is sensing the activity and the burning so your liver is giving out more glucose. 101 on exercise if bg is under 8.3 before your work out eat a 15 carb snack, if exercise is longer then 30 minutes check bg in the middle of exercise if there is still a drop, eat another or sip some sports drink, and 100% after the exercise if under 8.3 eat another or dring another 15 carbs.
I feel playing aroud with the lantus too much will cause an eventual rollercoaster, because the more you work out loose weight, your gonna probably start noticing the need for less lantus.
And one more note, never work out if your bg is above 13.8, this only makes the bg's worse.......

I've heard that you shouldn't inject more than 7 units at a time per location, meaning multiple injections to get the full dose. I try to spread larger boluses out, but it is easier with a pump.

Thanks! But it's not that. I monitor my BG daily and frequently, even after a workout I never go below 3.6-3.8 mmol. Haven't had a hypo for some 3 months

Nope, I feel very good and my BG never drops low. Problem arises 6-8 hours after the workout.
I wouldent say that would be the case that 16mmol/l in the first morning how are the other mornings are like?when I dont exercise anything between 4.5-6mmol. I don't go hypo at nite either, havent had a nite hypo for about 3 months. Nor do I go hypo at any stage during the day between my workout and the BG spike
