Did any of you receive a card in the mail to take a survey of the Cosmo and Smith-Medical services?

I received one yesterday and filled out the on-line survey. If you want to really tell Smith-Medical what you think about the Cosmo, the Cleo, the CozManager Software, etc., this would be the opportunity. And, it is completely anonymous. So, if you receive it, I suggest you complete it honestly.

I may have received it because I posed an inquiry to their engineers or because I got the upgraded pump but I filled out the questionnaire and submitted it on-line. I made several suggestions and gave my honest opinion of several of their products. They also wanted to know if I would recommend the Cosmo to others...I said yes.

Received the card yesterday and filled the on-line survey
today.

Yes I would recommend Cozmo and would choose it again,]
but Smiths does have issues needing attention in their
customer support/relation department.

They even had mistakes/inconsistencies in the survey, but I
gave them credit for the good intentions (now they need to
work on the execution).

Nope, I didn't. And darn it, cause now I have a whole new list of features I want to see added to the Cozmo.

I was positively surprised that I was contacted today, one day
after I filled the survey, by Smiths, regarding the issues we
have with the Cleo.

Smiths' rep claimed they hear very little about clogging issues
with the Cleo, so please, if you experience clogging - let them
know.

Also, inquired about the 1800 upgrade and they claim we are
in their 3rd group and should be getting the test email within
the next few days.

There was paperwork in with my sample Cleos, including a survey, which I filled out. I had clogging issues with the cleos, and both the ones I tried came off early.

I mailed the survey last week, but have not heard back from Smith's.
