View Full Version : Lantus, am,dinner or bedtime??

I have been on Lantus for about two years with Humalog. All is about to change...Paradigm delivered and awaiting training.
BUT>>>>>:o I still have a question regarding my Lantus...I have always given it in am 15 units, and then for some reason, can't remember now, MD changed it to 8U am and 7 u at dinner time.
My question is: I noticed most you take your Lantus at bedtime....is that what I should be doing???:confused: :confused:

I shoot Lantus in the evening. Originally, hospital told me bedtime... however, despite noticeable fade at 22h, there's still enough active after 24h that I experience glargine stacking. Not cool for shooting Lantus right before bed.

My mornings and afternoons tend to be unpredictable, or I'd probably split the glargine in two.

What should you be doing? Whatever works best for you. :)

When I took Lantus, my best results were taking it at bedtime, BUT I had to have a bg of at least 220 at bedtime, or I would go hypo at night. I tried splitting; some in the am & some in the pm, but never got it right before finally getting back on the pump. Whatever works best for you is right!

Congrats on the pump. It's been a little rocky for me, but I love it most of the time. Seems like I've re-learned everything the hard way (big bubble in tubing, cannula pulled out, "bad set"), giving myself a few highs, but overall I feel much more stable and happier. A little obsessed testing 10x a day . . . but nobody told me I had to do that! It will pay off I'm sure.

I've come full circle: was doing Lantus at bedtime (like Jan, I would snack with anything less than a 180). Then my new doc said let's try splitting it. That was okay. Don't remember why, but we then tried it in the morning. First visit to an endo in eons on Friday. She recommended going back to bedtime. Her reasoning was that if it wears off during the day, you (I) can correct it with Humalog. She said splitting was for much higher doses than I take (10u/day). But that's MDI and you'll be pumping soon. I wish you the very best.

Well, I 'MAY' be pumping soon...I'm trying to have an open mind...I have fought it for a while now..I don't mind MDIs and I do mind having something taped to my stomach...so we'll see how it goes.
I think my endo wants me to split Lantus since it was not going the whole 24 hrs for me, and that way it provides some overlap??

That's fairly common, from what I've gathered.

And then there are screwballs like me, using a little bit of NPH at appropriate times to cover the Lantus fade. (Actually, I've never heard of _anyone_ else doing that. But it works well for me.)

Whatever works for you is what's best. I don't think there is one "right" way.

I'm on Lantus, have been for 7 years, only just getting it sorted, but thats due to poor advice. I have mine at bedtime, alway have. Like Eddie, I use a little bolus, (humalog in my case) to correct the fade usually after 21 to 22 hours, (not always needed). It seems to be working for me. An am injection would cause a fade in the early hours of the morning, and a midday one the fade would be while I'm working, both not good times to correct, but as everyone say's, its what works for you

Ive only been on lantus for the last six months or so. At first i was giving 20u at night, and i was having major hypos over night (discovered after three day stinit on CGMS) so i split the does 14u night and 10u day, and its working really well for me so long as I bolus correctly to cover the food,.Otherwise i sit at 5.5 all day, but hey, we all need ot east right? :P

Hi- I take my Lantus in the evening, usually about 9:00 p.m. I am fortunate in that Lantus lasts 24 hours for me. Before meals I take Novolog. I used to take Humalog but than they quite making one half unit pens and so I went to Novolog which makes half unit pens. I really like the ability of taking half a unit extra occasionally.

I'd take some at dinner as mmy afternoon basal need is minimal. If it runs out, I don't care too much. I'm not sure if I would take any at breakfast. I don't think I would as I now ramp up at 4am for my DP. Hopefully I'll never have to find out.

what you *should* be doing is taking your lantus whenever it works best for you!

Personally i take mine 12 hours apart at 10am and 10pm. I have two alarms set on my watch to go off at these times so i dont forget.

Generally, if im high or low in the morning, i then adjust my evening dose, and if im high or low before lunch dinner or bed then i adjust my morning dose. It works quite well... for me.

I like mine split 50/50 12 hours apart - I feel it gives me the smoothest background dose. Like others have said though, you need to find what's right for you. (you, not your doctor...)

i take mine all at around 8pm. i think it covers the whole 24 hours, my most likely to go low time is around 5 in the evening, when i want to drive and go swimming (i know, typical!) so i presume its still doing something!

Glargine/Lantus stacking...

Although Lantus starts to fade (in me) after 22h, it's not completely out until well after 24h. The "new" injection ramps up faster than the old one fades, creating a window where I have _higher_ Lantus concentration, causing hypo tendencies. It's just like stacking a bolus, only it's happening with my basal.

Definitely bedtime for me. I am at 26--28 IU at the moment (``Honeymoon'' over!) and it works very well. I no longer the morning spike, it allows me to skip breakfast (Can't stand eating in the morning!) I bolus with Apidra.

Hi, I was started on bedtime lantus 3 months ago but was having 4-5 fasting LOWS a week so have changed it to 9am. Haven't had a morning low in 2 weeks. Only problem I have is remembering to take the shot! I finally got an alarm clock in the kitchen. I will probably going on MDI as most post prandials are 140-200. I think this is an ongoing learning experience!!!!!!!!!!

I take my Lantus in the morning, It works best for me to take mine in the morning, but I also take two pills to help control my sugar levels. After being diagnosed with diabetes, I was put in the hospital for a couple of days. They would give me my shots in the morning but when I was released they told me to take it at night. However with my job, it's easier for me to take it in the morning which is better for me.
