View Full Version : Double dose of Lantus! Doh!

Not recommended: Last night I managed to inject twice as much Lantus from 9 units to 18units. Now I imagine if someone else had done this they would have had severe lows, however my lows were gradual and less severe than I would have thought. I woke up throughout the night eating things. Could it be that I just gave my pancreas a "rest" for 24hours or is simply that the Lantus hasn't settled down? I read something recently that said if you’re taking less than 20units TDD then you are honeymooning (I am about 17 TDD), the reason for this conjecture is that I am trying to second guess my honeymoon, it’s seemingly towards the end if indeed I am honeymooning.

12:44am: 4.4mmol/L (not sure if I ate anything here)
1:49am: 4.7mmol/L and had bread 7.5g of carbs
2:41am: 5.5mmol/L
3:00am: 6.3mmol/L and had milk + muesli (homemade) 18g of carbs
9:00am: 8.5mmol/L

I can honestly say that I have given more Lantus than I was supposed to. And usually nothing happens except i wake up in the middle of the night with a little bit of a low, like 60 or 70 something. Nothing major.

But don't you LOVE Lantus?? I like the fact that it goes for 24 hours.
I say that because someone said there is no such thin as a 24 hour insulin.

I actually hate the stuff. It is evil stuff for those of us that drop hard in the middle of the night.

17u / 24 hours is .71u per hour and should be easy to cover. I'd probably cut back my boluses a bit and eat a cow or some other high protein snack before bed. I'd also be up at 2am testing and eating.

I didn't know that. See my problem is I always have high BS no matter what time of day it is, and being on the lantus really helps me, but hey we all have our own opinion.

I can't believe some of you take such small amounts, haaa I take 52units of Lantus at night.

I guess you are right, maybe I’m over analysing it :D I did exactly that cut my bolus back and had a few small snacks, frequent tests.

What is your average TDD and Insulin:Carb ratio(s)? Maybe you could make a new post or post here (i dont mind) and see if people can solve your high BS, could be a number of factors from basal (Lantus) to I:C to stress. Having a "normal" BS would definitely make you feel better ;). Just an idea.

In my case, a double dose of Lantus would be horrible...25 units doubled to 50 units...not a good thing. I fight low days even with 25 units and have to cut back.

I've had the odd occasion where I forgot if I'd taken my morning dose (and only dose)...I have gone so far as to wait about 5 hours to see if I'm going sky high...or not...I would never take a second dose...did that and worried all day (needlessly, it turned out)...

It mostly happens when I get up super early or when my morning routine gets turned upside down. A sticky notepad in my insulin box is good for jotting down a checkmark for the date...I need to go back to that routine the older (fuzzier?) I get!

When I did Lantus, it was 32-ish U/day. Each U drops me 35 mg/dL, or covers 15 g. To correct a double dose, I would have needed to eat 480 g of CHO during the following 24 hours.

What's nasty is when you run a needle of Lantus too deep and hit muscle. It took me four hours to get above 70; the next day, I couldn't seem to get below 150.

I suspect that your pancreas is still making some insulin. Increasing the Lantus dose just means that the remaining beta cells don't have to work as hard to keep blood sugar down. More insulin can also be stored in preparation for the next glucose load. So you may find that you don't spike as high after the next meal. Ideally, you want to titrate the Lantus dose up to the point where you start having hypos. At that point, as much of your endogenous insulin as possible is being used through the phase one insulin response to cover meals. It makes achieving stable blood glucose a lot easier. Enjoy it while it lasts .... ;)

what scares me is when i pick my my rapid pen by accident and dial up 16 units of that before bed. lol. i nearly always notice before injecting thought so it's never happened. Ill do it one day, i just know it.

Good to hear you weren't affects too badly.

Could you put some tape around the rapid pen, so that it feels different in your hand? I did it with the box for my contact lenses so that I could easily identify the right from the left (put red tape around the box for the left, thought red made sense for left)

You betcha, I have had done that mistake with Protaphane as I take 70U. One morning I have asked myself did I have my Protaphane I did but thought no and guess what... Yes you have that right I have taken 140U of protaphane!!! :eek: So I have a real job of getting my head above 5MMOL/L :( I just nibbled all day. It's kinda like floating in deep water and you test an hour after hour. :confused:

I'm not sure that the problem is with Lantus itself. The best that you can expect from a long-acting insulin is that it will give flat coverage. If that's what you need, it probably will work fine. However, problems will arise if for you need a lot less insulin in the middle of the night, or have a basal requirement that isn't flat.

Expecting Lantus to deal with that is like blaming your dog because it can't sing Mozart arias. The most you should expect is for it to be good at barking.

That is one reason I like my big, ugly, clunky OptiClik Lantus pen...it's so "dismal" that it rarely pick up my fast-acting pen by mistake. OptiClik is an ugly blue...Humalog is sleek white.

Nicole, do you exercise much? Taking a lot of exercise increases my insulin sensitivity to the extent that the amount of insulin I need is about half what it is where im not doing much. I recently had a Sports Clinic appointment and discussed my levels of insulin; apparently 10 units basal is quite normal for a type 1 of my height, weight and build(5 feet 5, about 140 pounds, muscly), whilst those with type 2 and significant insulin resistance can need massive amounts of insulin.....you dont look like a very big person and im surprised by how much you take, maybe you have quite a bit of insulin resistance or something? Maybe you could talk to your doctor about it and they might have some ideas?
