I'm confused by a few things here:

How can an Iceberg wedge have 15 carbs with no dressing?
How can a lettuce wrap have 82 carbs?
What do they do to Orange Peel chicken to make it have 200 carbs?
Baby back ribs, 200. Is that the sauce?
I like eating here, but I can't find much to eat!

IT depends on the size of the wedge. The wrap has other ingredients inculding dressings that contain carbs, try making your own! Many marinades have high sugar content so be careful of them. Lemon chicken or beef is better for carb content. Keep looking there are many low carb foods out there we just have to search them out and frequently resort to making our own or substituting a green salad with broiled chicken breast when eating out.

For the wedge, given the difference between the USDA for a head and whats given on the PDF I would say the wedges are brought in portion controlled shrink wrapped, which means also most likely have been sprayed with a solution of citric acid and dextrose to prevent browning, hence the extra calories/carbs.
For the lettuce wraps I'll bet there is a stir-fry type sauce on the filling, a nice sugar and corn starch filled sauce. Same with the Orange Peel chicken.
Ribs .. yeah .. its most likely in the sauce .. but you have to be careful, most "dry rubs" have brown sugar as a base.

The main thing to remember is no menu tells it all. Never are just the foods included in carb counts. If you ask the restaurant employes they will be as confused as you are!

What do you mean by "Never are just the foods included in carb counts."?
What else would be?

I think what was meant by that is sometimes they count the sides that come with it in there - for instance, a listing for a burger might have the calories/carbs for the fries that come with it too.

I'm not surprised, the calorie counts of those dishes are horrendous! The portion sizes must be huge. I think that you are stuck with choosing from the 'healthy' section. There's certainly nothing healthy about the rest of the menu.
What particularly horrifies me is the kids menu.
A cheeseburger has 855 calories and thats without the chips which have another 555.( total 1410) By comparison a Mc Ds Happy meal with a cheeseburger and a fruit drink has 545.
