Sliding Scale? What does it mean when someone says they are on a sliding scale insulin regimen?

I'm on a "sliding scale" for Lantus. What that means is that I check my fasting BG for three days, check the average and then adjust my Lantus dosage per the "scale" the Endo provided so that my fasting BG is in the 80-100 range. My understanding is that the actual scales differ for each situation, but for example if mine is running low (or sometimes just consistently 80) I will adjust down by 2 units. And the scale is based on the running 3 day average.

I'm on a sliding scale of Novolog which is my fast acting insulin. I take it before meals and it is based on a scale of; less that 100, 101 - 150, 151-200 and so on with each step having a different amount of insulin at each meal. Then I adjust that scale depending on if I'm exercising or on the amount of carbs etc. of my meals. Sounds more complicated than it is really.

I use my insulin to carbohydrate ratios and adjust depending on severity of and reason for the high. It's all very much shoot-from-the hip. Highs can be quite complex with causes ranging from miscalculations to bad sets to liver dumps to the positions of the planets.
