Has anyone come up with a good estimate for the carb count in homemade bread. I understand that it can vary greatly, but I've never been able to estimate how many "equivalent slices" there are in my slice.

This may or may not apply, but my CDE said that bread products can be estimated to be 15 gms/carbs per oz. This of course may vary some from one product to another. Your best bet would be to ck the packaging (if applicable) or get a recipe book that includes that sort of information.

I've tried to figure it out, too. The only formula I have been able to come up with (after looking at packaging on store-bought breads) is this:
Weigh your bread with a scale that measures grams. Whatever the weight of the bread is, figure that about 1/2 of that is carbs. For example, if you weigh a slice of bread and it's 50 grams, you should figure that it's about 25 grams of carb (or just slightly more).
This doesn't always seem to work if you take insulin, but I think it's maybe a timing issue more than a grams-of-carb issue. But if you're diet controlled, it may help to have this information.

I know the carbs for the types of bread I eat, So...........I just mentally compare the size/weight of the bread I'm gonna eat to known amount I, add about 5-10 carbs to be on the safe side, and eat away.
For instance, slice of bread I prefer is 18 to 21 grams of carbs. So I compare the density and size of the slice against the known size and density of what I normally eat, guesstimate the amount of carbs and then throw in 5-10 extra based on previous experience and Voila!!
