Anyone know of a good form used to track sugars and stuff?

Other than a meter, there are basic logs. I don't find them detailed enough. Perhaps this https://spreadsheets.google.com/ccc?key=tQp5VrYrxH00p9ezK4jlc7A&hl=en would be more to your liking. It's a spreadsheet I made.

I find layout and easy understanding, while keeping enough complexity to help reveal the relationship between things like food, insulin, activity, stress, other events, is key to getting the distilled messages out of logging.
I think John Walshes "smart chart" is excellent to consider, actually seeing the points or lines on a graph as well as getting the info can really drive home the issues. I'd put a star in the graph where my meals are, another symbol for injections. I'd probably want to work out a way to have room or place to write comments. It's often the step of writing down observations (from the logging and just from general observation) that actually gives me progress.

I was thinking of using John Walsh's Smart Chart -- got his book last night. It is so detailed and organized ... and I DO need something beyond my little spiral notebook, esp. if I move on to insulin.

Awesome, did you make that? How do you recommend I export that file and still keep it looking sweet? I guess my excel is sloppy.

I did make that. I made it, not with Microsoft Excel, but OpenOffice.org Calc (which is just like Excel). The one on my hard drive is nicer. It has more graphs. PM me your email address and I'll email it to you.
OpenOffice.org is a free, open source office suite. OpenOffice.org - The Free and Open Productivity Suite (http://www.openoffice.org/) I can use Microsoft Office, or Star Office, or GOffice, or KOffice.
Let me know if you have suggestions. I'm always looking to make things better.
