What do you do when you eat french fries. I've noticed that my sugar spikes 3-4 hours after I eat. How do you bolus for them??

I'm a pumper, I've used the dual method and that clearly didn't work, my bg spiked to 350 2hrs later. I've use the straight bolus of 6u and my bg spiked 3-4 hours later.

How do you bolus when you eat french fries or is that a no no food???

First ... I'm not a pumper. That being said, although I normally avoid French Fries; but I personally find then relatively easy to bolus for.

I use Humulin-R, which has a peak activity 2-3 hours after injection ... which corresponds almost directly to the absorption profile of the fries ... so assuming I calculate the carbs right, everything works out well ...

Are you definitely sure it's fries? Potatoes are high GI and should have finished metabolising into glucose long before 4 hours.

Shame on you, Fries I bad for you!

I have a hard time balancing my bolus with a burger and fries. I figure on about 10-15 units square bolus over 3-4 hours. Does not always work though.

Try around 10 units over 1/2 hour, than check in 3-4 hours. I have better luck with a shorter elapsed bolus and it kicking in over the next 4 hours as opposed to taking it over 3-4 hours. I always end up low with a long square wave. Maybe I need to strecth it over a longer time frame.

Look fwd to other pumpers suggestions!!:)

I think fat causes a bit of resistance along with slowing digestion. And then there's the problem with accurately counting something like fries. Add that to the rest of the meal...it's hard, and is probably why a lot of people avoid it. :( When I'm splurging on something like that I delay bolusing up to 30 minutes depending on what it is I'm eating with it and then proceed with the combo bolus. I also know that it's going to take a lot more insulin than normal, so I add about 50% to my regular bolus and I do a 20/80 or 30/70 split. Then I check in two hours and decide whether I need to make more of an adjustment or not based on what my blood sugar is and how much insulin I left.

Ray.......when you have used the Dual or Square wave bolus, did you give yourself the insulin all at once or did you spread it over a 3-4 hour period of time?

For instance, when it comes to Chinese food, pizza, or anything high in fats, I do a Dual wave and give 60% insulin up front and the rest over a 4 hour period. This seems to work for me every time!

Thats exactly what I do with any fried foods or potatoes that are fried. I use a dual wave bolus. The only thing I do different than KGM is I use two hours as the 40%.

Karen, I did dual wave 50%up front and 50% 3hours after. But 2hours after my upfront 50%, my bg was at 350

I'm gonna have to read up on the square...I don't know how that works...

Dual wave is really better for preventing lows than for dealing with highs later.. unless you add additional insulin to your bolus and extend that as the square wave portion.

I need 40-50% more insulin when eating really high fat foods.
