Hi Everyone,
Below is my "Medical Alert ID" card that I carry. Let me know if you have similar. If not, let me know what you think. Thanx.

Looks good to me. Sorry I can't be more help, don't know what kind of input you're after. Where did you get this card, or is this something you typed up yourself?

This is a prototype I made. Is there any extra info I should add for emergency purposes? I figure what's on the card should give firefighters, paramedics, police, colleagues, family enough to save me in the case of emergency.

More than enough.
If wanted to be 100% thorough, which you seem to be with even including your dietary intake - I'd probably add "Insulin Dependent" under the big red "Type 1 Diabetic". Why not make it ultra obvious you need insulin, that is the primary point along with contact info. Obviously the vast majority will realise dependency anyway, but you could be in a situation with a layperson who is hazy on their "types" and doesn't turn the card over to get the further clue of insulin dosages, etc. As ridiculous as it sounds, I've had people very surprised that I'm type 1, simply because I am in my 30s and in their mind eye type 1s have to be kids. (And I guess those type 1s don't get to grow up... ;) )

I did something a bit different. I came up with a form that fits an 8 1/2 x 11 sheet of paper. It contains ID info, Insurance info, List of medications, Medication Schedule, Allergies, Medical history, And all important Phone numbers (next of Kin, Doctors, Pharmacy, etc)
It has all the info needed to get me from Paramedics finding me on the street thru ER and Admitting.
I keep one in my wallet. One on the refrigerator, one in my medication box, and I print one out to take to every Doctor appointment.

More excellent ideas! Neat! I keep forgetting that I'm in Canada with "universal" coverage...thank everyone here for that! Since I carry my Canadian health card (in my wallet), no need to put my health number on the alert card? Last, this card will be folded over then laminated. I will make a dozen of them and distribute them to all in my surroundings with explanations. I can't afford another close call as I have had in the last six months. Thanx again.

Don't forget that people need to look in your wallet to find it (if they don't know you & you're not at home/work where they know.) EMS doesn't make a habit of this...perhaps put something on a medic-alert bracelet/necklace/etc to instruct them to look in wallet for more info.
