Eating out in the UK

Hi everyone :)
I'm a year 11 student and I'm doing a food tech coursework piece at the moment on Diabetes. I've got to do some research on how good restaurants in the UK are at providing healthy desserts for people with Diabetes. This includes a questionnaire to give to various people. Unfortunately I only know a handful of people with the condition and I need some more responses to include in my work.

If anyone wouldn't mind doing a short questionnaire, please email me at ------- and I can send it you. I won't use any personal information whatsoever, and plan to show my results in pie charts anyway to keep everything anonymous.
