I try to get as much fiber as I can. Even in large amounts, it doesn't affect my blood sugars. So I subtract all of them from carb total for the net carbs.
I will indulge in sweets with sugar alcohol every now and again. I try to stay away, but when I have the taste for something sweet, it suffices. And they don't affect my blood sugar either.
Fiber and sugar alcohol don't raise my blood sugar, but they do a number on your water bill if you know what I mean.
