Hi all,

I am trying to help my mom. She has been living overseas and back in September I was able to bring her to the USA to stay with me for at least 4 months. She was diagnosted with diabetes years ago but I do not think she has been properly trained or educated by her PCP to manage her diabetes. After a visit to the doctor here in th USA, 2 weeks ago, I became more interested on everything she has been doing, I also learned that her FG has been around 150 for quite sometime, even with twice a day Lantus insuline. Long story short, doctor changed the dosage and ask her to start with once a day with Lantus 15 units at night and finally this week (21 units at night) she has been "hitting" FG below 120 for the last 5 days. It's time to take her back to the doctor, he wanted to see her once FG < 120 was reached for a full week.

Since she eats between meals, her glucose level is NOT below 140 before lunch or before dinner. However, her glucose 2 hrs after breakfast and 2 hrs after lunch are below 180 which, from what I have read, I believe is OK. However her glucose 2hrs after dinner is around 200. I think thi is because the effect of the Lantus insuline does not last the full 24 hrs.

Question for the group, What are some of the questions my mom and I should ask to the doctor at this point? I believe her average glucose (AC1 ? ) was 7.9 back in September.
My mom also told me that before to come to USA, he tried Metformin but It was not good for her, she was lackking energy all day and fell like depressed. Any share of guidance on what questions I shoul be asking in my next visit it will be really appreciated.

The Lantus is background insulin and will have little effect on her post prandials. It sounds like she needs to be put on a basal/bolus regimen where in addition to the Lantus she takes a fast acting insulin before each meal, dose to be determined by carb intake. And by the way the ADA may say 180 after meals is ok, it's not. Diabetes complications begin around 140.

the ada is wrong but what they are saying is two hours post meal 180 is okay. and even then, it sounds like your mom may not even be there all the time.

diabetes is mainly a diet and exersize thing. ask questions if you would like. its about carb counting and talking a stroll. so alot depends on how your mom feels about that.

being over 100 like that alot is not ideal.

that being said, its a start to a good converstion with mom and dr.

she is type 2 right?

Correct it is diabetes 2
