I am going to see a new Endo on Friday. I want to have as many questions and information for him as possible.

1)All my BS values for the past 2 weeks (testing 7-9 xs a day)
2) All my doses of insulin
3) My meals/ snacks, and exercise regimine
4) All my labwork inc. my last A1c
5) My vision problems, such as the incresed sensitivity to light

What else?

You might want to take a few more than just the last 2 weeks worth of results. I usually take about 3 months worth, complete with averages for each month and trendline graphs.

I used to take 3 months worth of results, but really the doc never looked at all of them. I only take a week or two now, I think ideally a month is perfect to give an overview. What the doc is looking for is patterns.

Get your old doc to forward your new doc your medical records (they have to do this upon request). Then you don't have to worry about it.

as Deus points out, you might want to take more than just 2 weeks worth of B/G.

Any other Meds you are on

What is current levels of Insulins and what you're done to alter them over last 6 months to a year

Any physical traumas or things that might effect insulin regimine and activity levels

General health observations about yourself and what you've done to deal with them
