My doctor advised me on a formula that instucts me on how much insulin to take after carb intake. I failed to write it down.

The formula looks something like this:

(BS + Carbs) - goal BS / Grams per unit

Your going to have to call your dr back up. Everyones formula is different.

That is not the way I do it. Gina might do something like that.

I take 1 unit of insulin for 20 carbs, breakfast,
1/25 for lunch,
1/20 for dinner

Then if my blood is not in target I would take something like

200bg/ 1 unit of insulin
300-350bg/ 3units

Tony is right.

Because look how differen't mine is from his.

I take 1 unit/8 gr of carbs and mine doesn't change for each meal.
Then anything over 150 I take the number subtract 150 then divide that number by 28.

i do it like that too mac,

i take 1 unit for every 15 carbs anything over 130 i subtract and divide by 25 because my correction scale is 1:25

OK, The correct formula should read:

BS - goal BS(125) / correction dose (mine is 40).

But where does the carb intake come in the equation?

I think than carb intake doesnt come in the equation, because it's independant from your blood glucose correction.

For my part I calculate them separatly.
BS- goal BS (8 mmol) / correction dose (10 is my son's dose)

and

carb intake (g) * carbs ratio (g/unit of insulin, usually 30 for my son)

I am hoping for one formula that can be filled with BS, Carbs, and Correction Ratio at each meal.

Well my ratio is for 15g of carbs 1.5 units of insulin.
And for anything over 130 I take .1 for each 10 points over. This works for me. Like everyone said we are all wired different. When I first got on the pump I would worry but I have a really great DR. that says he is only my pump "coach" I know how I feel at times of highs and lows and I have the knowledge to fix it! He is there for guidance and assistance but only I know how I feel.
