Pretty much all diabetic supplies are either through the Pump distributer or by Prescription by the doctor to the pharmacy. some stuff is over the counter
Insulin vials, syringes, test strips, ect... - Prescription Pharmacy.
Pen tips, lancets, Glucose tabs - Over the counter

Here in Washington,
The only prescription only supplies are Lantus/Levemir and Humalog/Novolog insulin.
Regular, NPH, and other insulins can be bought without a prescription. Syringes, pen needles, glucometers, test strips, ketone test strips, and other supplies also are all over-the-counter and can be purchased without a prescription. If, however, you want an insurance plan to pay for these items, you must have a prescription for them.
Pump supplies are all precscription only and sold via the pump manufacturer or a specialized 3rd party distributor, usually via Mail.

In NY you can buy test strips, meters, up to 10 syringes at a time, lancets, and NPH, R or 70/30 insulin over the counter.
Anything you want insurance to pay for you need a script.
To buy more than 10 syringes at a time, Lantus, Levemir, Novolog, Humalog, Glucagon, or any of the oral meds, you need a script.

Actually it is different in every State because it's State Laws that control it. So you might be able to find the info on like 50 websites. Where do you plan to visit? Someone from that State would be better suited to answer your question.

As Harold mentioned, each state has different laws.
Here in RI, you can buy vials of insulin without a prescription, but you need a script for pre-filled insulin pens or cartridges. You do not need a script for syringes.
Meters & testing strips you can buy over the counter.

I know that in Tennessee and Georgia where I have lived I was able to get Lilly's R and L in the humilin line as well as the beef and pork with out a presciption for the last 2 years. Humalog needs a precription. with everything else no need for a prescription, with the needles however I was getting more than a couple of packs of 10 I needed something that shows I am a diabetic (my medicalert braclet usually worked for that)
as far as the cost of the insulin vials go I know about humalog now in my area. It was $42.00 USD the last time I looked which was 2 weeks ago

I think Novolog, Lantus, and Symlin are prescription-only in all the states by order of the FDA. In California, you can get syringes over the counter (OTC), but you have to sign a register for them. I've bought a box of 100 without a prescription. You can also get Humalog, Novolin, and NPH OTC.

In Texas.
Any Type of Insulin & Glucogon are considered "Prescription Drugs"
Syringes, Testing Supplies, Lancets, Glucose Tabs, Meters are all considered "Over-The-Counter" but you can get prescriptions for any of these from your doctor that will alleviate the costs.
I do think that Diabetics should be allowed "Under-The-Counter" Prescription Drug Coverage though. Just MHO :)
**Side Note**
I have "heard" that if you don't have insurance and you are diabetic that the pharmacy will give anything that you need over-the-counter with a small pharmacy discount. This is apparently arranged through the state and you have to fill out a **** load of forms.
