Chart for keeping up

I recently came across a chart I like but had to make modifications to it. When I took it to the doctor he liked it as well since you can lay them out side by side on a table and look at the trends. He know has his copy of it and I am sure hands it out! Wanted to share it with you. If you for some reason can't get it to copy/print like you would like PM me and I will send it to you in an email.

Belinda, thanks for sharing the chart. It is almost exactly like the log book that Disetronic provides except for the bottem part which is blank.

I use the blank part to put in any information that may help explain why the numbers are either up or down or in my case somedays erratic...like last night when I forgot to bolus for some hot cocoa

I have looked over both charts and they are both impressive. I, a little bit, wish I was as organized as the two of you are.

It's nice to see that some of y'all are doing this the correct way

I'm afraid, however, if I became that organized my wife would be convinced that the "Pods" had replaced me.

LOL RZR... I only keep a chart 2 weeks prior to the visit...or when I need to see what is going on. I just hated the books and the other charts I have been given . This one makes the most sense to me and I modified it for me so it better make sense..

Basal is the insulin you inject/pump to maintain your baseline.

Bolus is what you inject/pump to cover carbs taken in or to bring you back to proper baseline.

Nice log. We like to keep one week on a single sheet. I have the original in an excel file with formulas for averages and dates.
