| | | Need basal advice My bg around 9am is now always about 20-30 above my pre-prandial target. At what time should I increase my basal to bring it down? In otherwords, how many hours ahead?
I'm pumping Apidra. I know I need to work on it but need advice on where to start. Would it be a little increase every hour for the 3-4 hours prior?
Would really appreciate your feedback.

Start with maybe 1 - 3 hours before you normally wake up, and go earlier if you have to. (I'm assuming you've already tested as soon as you wake up to rule out the 'wake phenomenon' version of DP...?)
But as I said, I'm not a pumper and don't know much about it! Just seems like the sensible trick.

| | How about your 8am, 7am, and earlier numbers? Perhaps you are high all night long? I would be basal testing as much as possible until I painted a clear picture.
Apidra runs about 6 hours with a spike one hour in: Aventis Pharmaceuticals -Apidra - April 2004
Go slowly. Keep in mind that 1u at a ratio of 1u:10g of carbs should drop your BGs 50pts. It sounds like you may need a half a unit. But it could also be a timing issue. Adding 1/2u could drop you low anywhere in 10am to 1pm. I found some adjustments are simply moving basals earlier or later to even out the waves.

| | | Normally I say 2-3 hours. However, Apidra works alot faster, therefore I would say only 1 hour ahead of when you want the change to take effect.

| | | I've done alot of testing and my numbers are consistent throughout the night holding at around 95. I'm about the same at 6am, rise slightly at 7am, rise some more by 8 and by 9am, I'm much higher than targe.
So the spike happens within 1-1 1/2 hours starting around 8 so by the time I bolus for breakfast at 9 I've gone from 95-100to 125-140.
