Unhappy Problem with blockage in tubing?

I have had this Cozmo1700 pump for about 6months and replaced. I have used Silhouette, Quick-sets, and now Rapid-D. I have even changed from Novolog to Apidra insulin. I leave the insulin out to room temp. prime the infusion thing and all that good stuff. I even had a pump trainer come down to see what the problem was and we are all puzzled?
It would work for about 3 months no problem then bam! I would get 1 to 2 inch blockage in my tubing preventing me to get insulin and causing high sugar readings. When I had it replaced it would work for about 3 months and then same thing. What is up with that?
I am now on the shots. I even tried to have it on my freaking hip which bothered me a lot. I usually carry it in my pocket. I have only been on this pump for 1 year. Dec will make 2yrs. I don;t understand what the problem is. I am very petite, lean person and very, very active. Any suggestions.
