Are you 100% sure that your basal rate is correct? It will do no harm to test your fasting bg at various times pf the day over the next couple of weeks. If that is not correct, you can count as many carbs as you like and your bg will still not be predictable. Been there

If it is out, your endo should be interested in the results and advise if change is needed.
Also, gastroparesis. I know little about this, but isn't it the case that it makes food absorb more slowly? If that is the case then I would have a chat with your endo about the Apidra. I'm now on Apidra and, whilst I love it, it comes on like a charging rhino with no mercy. I have a really fast digestion, and for some foods it is too fast for me even with my fast digestion. On some occasions, I've had to hit the orange juice 45 mins after a big meal. The carb count has been correct, but the insulin has a race with the food and as this stuff likes to pretend it's Carl Lewis you need to be aware of that.
There are two 'rapid' insulins on the market that are billed as being slower (Novorapid or Humalog) and there are obviously some of the older style insulins such as Actrapid. I was on Novorapid for years and it is a lot gentler than Apidra (at least in me it was

) If your basal IS correct, then discussing with your endo as to why Apidra is best for you considering your stomach problems may be beneficial.
