High BS at night.

I have had type 1 for about a year now and I started to have high BS at night and I only take one insulin (lantus 23 units) before bed. They used to be fine but now they are getting to high it scares me. I have not changed anything for it to be high at night. My last A1C was 7.0 and I feel like I do not know enough about this yet. I live in a very remote area and do not have specialist around to help like I wish.

Patience, my friend, patience....

It could be that your Lantus isn't enough at night, and you need to increase it. But in order for anyone to help you, you'd need to do some testing in the middle of the night, and post what your numbers are, and when.

I have done that and they would level out to around 180 at 3:00 and in the morning on wake up they will be right at 100 and some have been in the70s

well, i would say if you aren't eating before bed, or bolusing with a rapid insulin, and you are waking up at 100 or less, then you shouldn't worry about hitting 180. 180 isn't high at all, in my opinon.

It looks like your body just rises naturally while you sleep, and then your Lantus is dirving it back down by the morning......

I know 180 is not all that high, but at before bed it rises to around 250.

What time do you take your Lantus and what time do you notice the high bg? It sounds like the Lantus may not be lasting 24 hours. Some people need to split the dose and take it twice a day.

Rising in the night like that typically has a cause other than basal release. A carry-over from dinner or late snack perhaps. Where you wake up normal, your Lantus has no room to move up. If you correct this high, you may need even less Lantus.

Do some detailed testing in the evenings if you can. Test every hour from just before dinner till bedtime on a typical night (no cheating, be normal) and see what's happenning. Also write down everything you eat. One suspect is high fat and/or high protein meals like pizza or chinese.

Another huge suspect is your Lantus running earlier than 24hrs. You may need to take it earlier and/or split your doses. It's much better for it to run out in the afternoon than the evening as you typically don't need much in the afternoons.

A good test is to skip dinner and test hourly. If you continue to rise, it's your Lantus. If you don't, your mealtime insulin is off. I think

I've screwed up too many times by assuming. It's always best to test to prove your theories if you can.

I had that same problem, it just means your doctor is going to have to increase your Lantus a few units. I went from 23 units when I was first put on Lantus, a couple years ago to 50 units, which is what I am at now. I mean it didn't increase all at once but over the years it has, and every few months they have to increase it until it finally stabalizes, then they lower my dose until it needs increased again. Its nothing to sweat about, just let your doc know your worried about it and see if they can increase you maybe a unit or two.

That is good advice. Thank you. I was not aware that one can take that much insulin. I thought that I was on the high end. Are you taking all of that at one time? Do you use the pen or regular syringes?

Also remember that Lantus has a 4 hour "warm-up" period before it reaches it's flat-line (nearly) level.

Are you taking a short-acting (Humalog) at meals...I assume you are since you are Type 1. Are you getting a good 2 hour reading after dinner...then rising up to 280's?

I like to take my Lantus in the mornings so I'm fairly flat at night...even then, I drop about 50 points so I need to snack before bedtime. I normally have good readings in the morning.

I am only taking Lantus, I thought that I needed to be fast acting but I have not been put on one. I am nornally have a decent # after dinner but progressivly goes up.
