questions about changing basal needs

Since I've gotten over the infection and finished up the antibiotics my numbers are still not quite right. I saw the doc today and he said I'm all clear of the infection so these numbers are an indicator of a need for a change.
I can look at the patterns and rather plainly see what I need to do. I've got to increase the lantus at night (bs going up 30-50 points over night...was remaining flat a ways back) and I possibly will need to decrease the AM dose...I've had a few hypos lately in the early evening long after the humalog from lunch has disapeared.
My question is this: I'm going to make some adjustments...do some testing...but at what point should I do basal testing again? After I'm seeing better numbers, or should I do it now, even though I'm pretty certain I know what I need to do (doc looked at my numbers and agreed with me, btw)?

Also, how often does one's basal needs change? Just curious.

My rule of thumb is go slow, like I do that. Pffft!

I do question his conclusion that you need changes. (Playing 's advocate) Has enough time gone by for you to be back into your normal routine? Have you ruled out all stressors?

I take most everything they say with a grain of salt, so question away! Things have been normal (whatever that is) for the last 5 days and I've seen a consistant pattern. Corrections aren't even working normally...and my conclusion was that I need to make a basal change.
As for ruling out all stressors, well, there are a lot right now, infections aside...it's been that way for months and I have no way of knowing when it will stop. Being high all the time is causing me untold frustration as I have not let a single solitary carb cross my lips without the insulin to back it up...and the numbers still suck! That stress alone is enough to make me start jacking it all up...at this point I'd actually welcome a few hypos. Sad, huh?

The hard thing is, as you may know from my "confession" thread a few weeks ago, I haven't really been keeping good track for months...I may have needed this adjustment all along and in my neglect, missed it. I'm halfway wondering if my poor control all these months was why I got the infection in the first place, but that's neither here nor there. Not having anything decent to look back on at the moment isn't really helping. I know that my current routine worked in June...and I did basal testing and confirmed it.
Since then I lost my mind, lost control and gained 8 pounds. And it's February, not June and I seem to dimly recall that I've had to make adjustments before for cold vs. hot weather.

If you've got anything else to throw at me, shoot. I'm about as wide open as it gets these days.
