What!??

This is a typical issue i keep having...

I haven't had anything to eat since last night at about 9 (when i finished eating).

This morning i wake up at 6.8.
45 minutes later after a doctors appt. im 8.3.
I take 1unit of insulin after that test to correct. If it had been coming down i wouldn't have corrected. but it was going up.
1 hour later test again, 9.2!!

Still haven't had anything to eat at this point!

making no sense to me...

Stu,

I'm T2 and don't use insulin, so feel free to completely ignore this as it probably doesn't make sense. Is there any chance that it was the stress of the Doctors appointment "building" on your 6.8 FBG high?

Alternatively, is your insulin okay could you try another vial/pen?

i wouldn't have thought it would be stress... i wasn't stressing about the doctors appointment.

I've thought about a duff cartridge... but this has occured a few times before on different cartridges so i think i can safely rule those two out.

Maybe dawn phenomanon is sneaking up on me? Apparently the pump can fix that... so in a way i hope it is because it gives me another reason to have a pump.

But it's annoying taking insulin and seeing my number go up still. it did come down to 8.8 an hour later... but that's still too high

Im hoping a brisk walk to greggs for lunch will bring it down a little, it's about half a mile there and back.

Possible reasons:
  1. Liver dump before you woke up, thus causing a mad spike.
  2. Not enough basal insulin.
  3. Dawn phenomenia, but you can sort that by increasing lantus. If that makes you low in other parts of the day you could start using a 12hour insulin like NPH to deal with the DP taken at bedtime.

Only way to find out is to test at 3am and do a basal test in the morning, i.e. skip breakfast and test every hour up till lunch time. I have been having my own fun with my basal dose at the moment, on another note if you don't figure it out then your insulin resistance will go up and you will probably require more insulin anyway. Happy hunting.

Somogyi Effect

The tendency of the body to react to extremely low blood sugar (hypoglycemia) by overcompensating, resulting in high blood sugar. The Somogyi effect, also known as the "rebound" effect, was named after Michael Somogyi, the researcher who first described it.

When blood glucose levels drop too low, the body sometimes reacts by releasing counterregulatory hormones such as glucagon and epinephrine. These hormones spur the liver to convert its stores of glycogen into glucose, raising blood glucose levels. This can cause a period of high blood sugar following an episode of hypoglycemia.

The Somogyi effect is most likely to occur following an episode of untreated nighttime hypoglycemia, resulting in high blood sugar levels in the morning. People who wake up with high blood sugar may need to test their blood glucose levels in the middle of the night (for example, around 3 AM). If their blood sugar level is falling or low at that time, they should speak with their health-care team about increasing their food intake or lowering their insulin dose in the evening. The only way to prevent the Somogyi effect is to avoid developing hypoglycemia in the first place.

if you can wake me up at 3am then i applaud you lol.

Once im asleep, im asleep, dead to the world, would sleep through a thunderstorm in a tent. lol

Im hopfully going to be fitted with a night time monitor to find out what my blood sugar is in the night, because i find it extremely difficult to wake up in the middle of the night.

i think it's hypo related, which suggests too much basal, but when i wake up high i automatically think 'oh, need more basal for the night'.

i hate this stupid diesease!

I would guess your liver is dumping glucose from not eating for awhile.

As, Funnygrl says, that glucose is coming from your liver. The longer the gap between meals, the more likely it is that the liver will produce some supplemental glucose. This why a lot of people snack before going to bed. It moderates that early morning blood glucose surge. In your case, you hadn't eaten since supper the previous night. Blood glucose will also go up if you are running low on insulin.

I thought it was pretty normal for people not to eat while they sleep !? lol Yeh ok most people eat breakfast... i usually don't (i did this morning).

I dont like the idea of having to eat before i go to bed, it's like feeding my diabetes just like it was like on mixtard30. I'd much rather have a pump and adjust a basal rate for it lol.

I woke up fine this morning, 5.8. But then who's to say i still didn't have a hypo in the night and i was in the middle of rising blood sugar... that's why i had breakfast this morning... i had enough time to, which is unusual and it ment i could take some insulin and hopfully nip it in the bud before it got too high of it's own accord.

I don't eat before i sleep, just adjust your basal so that you don't have to. I would start having breakfast if i was you as it sets you up for the rest of the day. Only way I wake up at 3am is if I am well rested or I have stayed up all night watching movies or something.
