Insulin Question....Correct or Not?

Many times when I have fasting blood work my blood glucose has readings up to 150 or more. Even though my regime for insulin is Humalog and Levemir I will go higher the later I have this test.

My gastro flagged this and sent it to my PCP and particularly wanted to point it out to me. Like a dummy I didn't say anything as I am used to it. Yes, I frequently have the famous dawn phenomenom.

My question is......should I ignore this and present it as always or should I keep testing and do a couple of units of Humalog to bring it to a normal-like number before the test? I never thought to do that before because I thought they would just want to know what it was at that particular time.

My thinking is that we should strive to achieve the best BS control possible... within reason... allowing for real life... and while avoiding going hypo If DP causes you to be high in the morning (every morning) then perhaps your BS control could be improved. Perhaps your insulin times and or doses need adjusting; plus some folks find a protein snack before bed or even a glass of red wine can help combat DP.

I don't have blood tests to keep the doctors happy but to help me try and prevent complications

Best way to control DP is with a pump. You can have variable basal rates based on time and this can eliminate DP.

This is all basal action.

Do you omit your Levemir the night before bllodwork? No! Take the humalog. You need it. Use this as an opportunity to figure out how much you need to level out your DP. Also discuss other options with your PCP. Perhaps a split of Levemir and NPH might be a betetr choice. Do you know how you behave between midnight and 6am? Get that figured out before making such basal changes. Humalog seems ok as you are awake and have decent tracking so far.

Going forward you should be aware that you have a fixed component to your morning bolus. Those two units should be counted seperately from your normal ratio.

I always correct a high before blood work is done. Mainly because my endo's have always hated seeing high fasting numbers on the lab sheet. No matter if it happens once a year...it will happen the morning you are getting blood drawn!

But, since you are not eating...don't over correct.
