Question Question for T2s on Humalog

I'm currently up to 40 units of Lantus each night and 1000mg of Metformin at breakfast and supper...

Tonight I cut my Lantus in half, and tomorrow morning I start Humalog injections at every meal...

My Endo has told me to inject 5 units with each meal for the next 3 weeks and then we'll review my logs and establish a starting ratio.

Considering that my meals almost never go over 45 carbs 5 units seems high to me...

What are YOUR I:C ratios???

*Humalog Bolus I:C 1:13
*Humalog Corrections I:BGl 1:40
Metformin - 1000mg at breakfast and supper

Testing 6 to 10 times a day depending on how I'm feeling.

*Using Vials and Syringes. My ins will not pay for pens or pumps.
They figure, if it was good enough for Grandpa, By God it's good enough for you!

When I started using the regime you are using they started me with 1 unit to 10 carbs which slowly had to be changed to 1 unit to 5 carbs. So if I were to eat 45 carbs now I would take 9 units of Humalog.

Just make sure you are eating those 45 carbs and test, test, test. There has to be a starting point. If you feel like you are getting too low and have to correct with some juice or other carbs I would call the doctor.

Sounds as if you are a bit nervous about starting this but don't be....JUST TEST !!

It took a while to tweak mine to proper units. I wish I were as disciplined to just do 45 carbs per meal but as I said they fluctuate....hence the different way for me.

Mine is 1 unit of Humalog per 5 carbs and 46 units of Levemir nightly. We are all different.

Type 2's can require alot more insulin than type 1's so 5u may not be that much -just follow the endo's instructions and carry some sweets/juice/glucose tablets just in case you go hypo

As an example of "Some" T2's needing more insulin, my regime is a good example

Humulin-I (aka Humulin-N to USA peeps)
100u twice daily

Humalog
2u per 1gram carbs
average total of 250u to 300u a day

TDD = 450u to 500u

But I have the added problem of constantly takin steroid tablets (betamethasone tabs) in high doses so am constantly fighting that BUT alot of T2's have insulin resitance and that can make the insulin needs bigger

1:10 seems the common starting point.

Eating the same meals (carb count and GI) daily will help.

I was taken off of all pills when I started insulin. I take 58 units of Lantus every evening and for every 15 grams of carbs I take 6 units of Humanlog. So if I were to eat 45 grams I would take 18 units of humanlog.

Poppa ,i don't think it's to much. But everybody is different "I started at 1 unit per 10gr now 2 units per 10gr. Some people take a lot of adjustment before they get the right dose '"Like almost everyone told you Test & test is the solution.Bye Ricky

Thanks everyone for the responses

I started this thread because I was afraid that a instruction to inject 5 units at every meal regardless of meal content was kinda scary....

I'm used to occasional spikes, but the thought of the potential lows had me worried...

Obviously, as you all pointed out, the answer was to test, test, and test some more...

Last night for supper I had a Lean tenderloin steak, half an avocodo (128grams), and 192 grams of steamed artichoke quarters, for a grand total of 10 carbs!

At the same time I took my 1000mg of metformin, and shot up 5 units of Humalog and proceded to test every 30 minutes for the next 2 hours...

BGl Before 1st bite : 150
+30 minutes: 138
+ 1 hour: 130
+ 1.5 hours: 132
+ 2 hours: 128

After which I had my 10 carbs worth of Medicinal Dark Chocolate I eat every night, and quit worrying...

Obviously my bolus needs are going to be higher, and this 5 unit per meal 3 week trial will just give the endo some baseline data to set up my initial I:C ratio...

Good Luck Start out low and test,test,test every thing you eat b4 and 2 hours after.even when you wake up and see that you may be high a correction may be required.the only thing i can say is test alot and write it down.

I eat 2-3 carbs per meal (15g=1carb).

I take Novolin and Humalog.
Novolin 32u night 20 u morning
Humalog 8 u breakfast, 14 u lunch, 12 u dinner

So.... it can also depend on your resistance during the different meal periods.... YES your I:C can be different different times of day, ESPECIALLY when you are on another type of medicine, i.e. metforming or long acting insulin.

Test TEST TEST and TRACK those #'s!!! Good luck.
