Ketones in blood and urine

Hello,
Just a question about ketones in blood and urine...What does it mean?
Just a little background. I went to the quick care on Friday as I was sick since Tuesday. (cough, runny nose, chest hurting). They sent me to the E. R. since my blood sugar was 382 without eating since the night before. My pulse rate was also 146. When at the hospital, they did a mulitude of tests. CAT Scan, EKG, blood, urine, & xrays. They gave me insulin to bring my blood sugar down. It went down to 263 after about an hour. They used the IV dye when they gave me the CAT Scan (which I instantly had an allergic reaction to). Well, they told me I could not take my diabetes meds since I had that dye until after I follow up with a blood test with my physician. I do have an appointment today. I just checked my blood sugar and it is 445. Oh, my original question was What does Keotones in blood and urine mean? They did tell me but I was high on the benedryl they gave me for the allergic reaction so I didn't really hear anything they were saying.
I just don't know how to get my sugar under control. I know that I have diabetes and I know I need to be a better diabetic, I just don't know how. Cut my arms off so I can't shove food in my mouth although I am sure I would find a way. Thank you for letting me rant. Any help out there?

Having ketones just means that the body is burning fat. Small to moderate amounts of them are normal if you are losing weight, haven't eaten for a while, or eat low-carb. But large amounts are dangerous and can lead to heart failure. DKA (Diabetic Keto Acidosis) usually only happens to T1s, and it is caused by lack of insulin. The patient is rushed into intensive care and put on a heart monitor, while IV insulin is used to bring the blood glucose down. It sounds like you may need to go on insulin at this point.
